Corrective actions and 8D
Every corrective action request follows the eight disciplines, D0 to D8. Doctilla keeps containment separate from the permanent fix, pushes back on weak root causes, and won't let a CAR close until someone has checked that the fix worked.
Training video 7: Corrective actions and 8D · 3:20 · chapters and transcript
What it does
- D0 to D8, step by step
- A clear stepper through team, problem description, containment, root cause, corrective action, validation, prevention and closure, saving as you go.
- Root-cause tools
- 5 Whys, Ishikawa (fishbone), cause and effect, and TRIZ prompts, with suggestions you can accept or dismiss one by one.
- Weak causes flagged
- Doctilla spots root causes that blame people or say 'human error', and asks why the system allowed it, including why your checks didn't catch it.
- Effectiveness checks
- Set a measurable check, such as a KPI over three months, and record the result when the time comes. The result is entered by a person, never moved by a number.
- Supplier responses
- When the cause sits with a supplier, record their response and your decision on it inside the same CAR.
- Proposed CARs
- A drafted CAR is 'proposed' until someone agrees it, and proposed CARs are kept out of every open and overdue count.
